Effective October 2026. This notice covers information you share through lumenbehavioral.com. If you become a client, our Notice of Privacy Practices explains how we protect your health information under HIPAA.

What we collect

Waitlist: your name, email, phone (optional), state, and whether you're a future client or referral partner. Careers: your name, email, phone (optional), license type, license state, and the role you're interested in. Please don't include health details or client information in either form.

Site visits: we use Cloudflare Web Analytics to count visits and see which pages are viewed. It does not use cookies or identify you.

How we use it

Only to contact you about Lumen's opening, a referral partnership, or a role with us. We don't sell or rent your information, and we don't use advertising trackers.

Where it's kept

Form responses are stored in Lumen's workspace, with access limited to Lumen staff who need it. The site is hosted by Netlify, which keeps standard server logs. Fonts load from Google Fonts. Visit counts are processed by Cloudflare.

How long we keep it

Waitlist information is kept until you ask us to remove it, or for up to 24 months. Applicant information is kept for up to 2 years.

Your choices
