Effective November 1, 2026. This notice applies to clients of Lumen Behavioral LLC. For information you share on this website, see our website privacy notice.
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our promise
Lumen is required by law to keep your health information private, give you this notice, follow it, and tell you if a breach affects your information. Where Georgia or Virginia law gives you more protection than federal law, we follow the stricter law, including Georgia's mental health privilege (O.C.G.A. § 24-5-501) and the Virginia Health Records Privacy Act (Va. Code § 32.1-127.1:03). We do not sell your information or use it for marketing.
How we use and share your information
- Treatment. Your therapist and Lumen's Clinical Director use your information to provide and coordinate your care. We share with other providers only with your written permission, except in an emergency.
- Payment. We use limited information to bill for services. If you pay in full yourself, you can ask us not to share information with your health plan, and we must agree.
- Health care operations. Quality review, training, licensing and compliance, using the minimum information needed.
- Business associates. Vendors such as our health record and email providers, who must sign agreements to protect your information.
When the law requires or allows sharing without your permission
- To prevent a serious and imminent threat to your health or safety or someone else's (including Va. Code § 54.1-2400.1).
- To report suspected abuse or neglect of a child, older adult or adult with a disability (O.C.G.A. §§ 19-7-5, 30-5-4; Va. Code §§ 63.2-1509, 63.2-1606).
- In response to a court order or as otherwise required by law. In Georgia, communications with your therapist are privileged in court unless you waive the privilege or the law makes an exception.
- For health oversight such as licensing board investigations, required public health reporting, and to the U.S. Department of Health and Human Services to show we follow HIPAA.
When we need your written permission
- Psychotherapy notes (a therapist's separate private notes), with few exceptions.
- Any marketing use or sale of information. We never do either.
- Any use not described in this notice. You may revoke permission in writing at any time, except for actions already taken.
- Extra-protected information: HIV/AIDS information under Georgia law (O.C.G.A. § 24-12-21) and substance use disorder records from federally assisted programs (42 C.F.R. Part 2), which will not be used against you in a legal proceeding without your consent or a court order.
Your rights
- See or get a copy of your records, paper or electronic, usually within 30 days, for a reasonable cost-based fee allowed by federal and state law.
- Ask us to correct information you believe is wrong or incomplete.
- Ask for confidential communications, such as a different phone number or address.
- Ask us to limit what we use or share. We must agree to the self-pay request above.
- Get a list of certain disclosures we made in the last 6 years.
- Get a paper copy of this notice and choose someone to act for you (a health care agent or legal guardian).
- Be notified if a breach affects your information.
Complaints
Contact our Privacy Officer, or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/complaints or 1-877-696-6775. We will not retaliate against you for filing a complaint.
